Change in Process for Accessing the Shower Insert and Ramp Kit

From today, EMS Assessors will be able to request the Shower insert and ramp kit (available from Multifit), using the Complex Equipment Application Form.

Previously, these requests were processed using a Complex Housing Application Form. However, shower inserts are not attached to the home and are not a housing modification. Housing Contractors are not required to be involved in the process.

For a period of time, all Complex Equipment service requests for the Shower insert will be referred to the Professional Advisors for review and consideration. The document attached outlines the type of information that needs to be included in your service request.
