Updates to EMS Advisory forms

Later this week, Enable New Zealand will be rolling out multiple updates to EMS Advisory forms on our websites, as well as three new guides for your reference. These are outlined below.

Updated forms

The following forms have received minor updates. They can be previewed via the links provided.

The updated ENAE211 Outreach Service Referral and Outcome Summary consolidates the following forms:

  • ENAE211 Wheelchair & Seating Outreach Service Outcome Summary
  • ENAO801 Wheelchair & Seating Outreach Service Referral Form
  • ENAO802 Positioning Outreach Service Form

Rollout timeline

These forms and guides will go live on our websites on the 27th August 2020. The links above are for your reference and to give you a preview of the upcoming change.

In order to facilitate a smooth transition, we will continue accepting the old forms until the end of September 2020, however they will be unavailable on our websites after Thursday. We would encourage you to begin using the updated versions of the forms as soon as possible. Please delete any saved forms from your desktop and if possible always use the most up-to-date documents on the Enable New Zealand website.
