Equipment Service Request System

Our equipment service request system is for EMS Assessors and service-accredited DHB staff to request Ministry of Health funded equipment for disabled people.

Accessing the system

EMS equipment assessors are provided with login details on registration with Enable New Zealand.

DHB staff are provided with login details on completion of their DHB’s service accreditation training course.

Requests for complex equipment

Before submitting a service request, EMS assessors must:

  1. complete the EMS Portal
  2. submit a completed EMS advice outcome summary, if required
  3. gather any other other supporting documentation such as quotes.
