Equipment Service Request System

Our equipment service request system is for EMS Assessors and service-accredited DHB staff to request Ministry of Health funded equipment for disabled people.

This system replaces Enable Online RTL from 22 October 2019.

Accessing the system

Use your login details provided via email on 22 October 2019, or on registration if after that date.

Here is a link to the ENZ equipment system:

DHB staff: If your DHB has implemented DHB Service Accreditation and you have not received a login, please check with the DHB manager who has oversight within your DHB.

The system will be available from 8am on 22 October 2019.

Requests for complex equipment

Before submitting a service request, EMS assessors must:

  1. complete the EMS Portal
  2. submit a completed EMS advice outcome summary, if required
  3. gather any other other supporting documentation such as quotes.
