Housing Outreach Service

Enable New Zealand has been contracted by the Ministry of Health to provide a Housing Outreach Service in various centres. This follows the success of the Seating and Wheelchair Outreach Service which has now been functioning for several years.

This service has been designed to provide a facilitation/support role for eligible complex housing assessments, before the detailed plan is to be submitted as a service request.

Credentialled Housing Assessors may request an outreach appointment where:

  • The client’s needs are extremely complex and/or
  • The environment to be modified is challenging and/or
  • The scope of works is likely to be particularly expensive

This Service offers:

Clinical support and advice from a coordinator who is a highly skilled Professional Advisor

  • Assistance in exploring all possible options with the client and their family/whanau (including housing modifications and other options)
  • Technical support both at the clinic and during the design process from a Housing Advisor
  • A mobile service in various locations ie we will visit the client’s home with the EMS Assessor
  • An opportunity for learning: new colleagues can attend to acquire skills and knowledge
